Medication Reconciliation Project Reflection

When working on the medication reconciliation project, I learned a lot about the importance of reviewing a patient’s medications as well as patient education in general. For this assignment, I had a patient on five different medications, two of which were over the counter medications, in which I was able to compare them as well as research each individual medication and make a plan for patient education. In terms of safety concerns, this patient had a few minor ones but the overall major safety concern I came to find was the risk for excessive drowsiness. Three of the medications the patient was on had drowsiness as an adverse effect which lead me to include this in my patient education as well as advise the patient to not drive while taking these medications. However, since my patient was an adolescent, none of the safety concerns I had for this patient were due to not being able to keep up with the medication regimen or any other physical restrictions that may come with age. For this reason, the patient was also able to explain why they were taking each of their medications and how to take them as well. This is important because it means the patient is coherent enough to explain to another provider why they are on the medications they are and understand the risks that could be associated with not taking the medication as prescribed. As for the type of medications the patient was on, they did take two anti-seizure medications in which the patient understood that the intended reason for this was to progressively move the patient solely onto Lamictal and eventually move off the Keppra, though for other reasons they patient has continued to stay on both medications per providers’ order. Luckily with the five medications the patient is taking there is little to no interaction between any of them in which the only major concern I had was the adverse effect of drowsiness in three out of five of the medications the patient was taking. Overall this project allowed me to have a better insight on the purpose of performing a medication reconciliation on a patient and the importance of understanding drug interactions and patient teaching as well.
